Customer Survey Sweepstakes Rules

The Genesis of Ontario Customer Survey Sweepstakes (“Sweepstakes”) is sponsored by Genesis of Ontario (“Sponsor”).

Eligibility

Open to legal residents of the United States, including California, who are 18 years of age or older at the time of entry. Void where prohibited by law. Employees of Sponsor, its affiliates, and their immediate family members (spouse, parents, siblings, and children) and household members are not eligible.

Sweepstakes Period

The Sweepstakes begins upon survey availability and ends at 12:00 p.m. Pacific Time (PT) on Monday, April 27, 2026 (“Sweepstakes Period”).

How to Enter

To enter, participants must fully complete the official Genesis of Ontario customer survey consisting of 18 questions during the Sweepstakes Period.

• All questions must be completed to qualify
• Limit one (1) entry per person

No purchase or payment is required to participate.

Prize

One (1) Grand Prize will be awarded:

• One (1) $500 gift card
• One (1) Los Angeles Dodgers gift bag

Total Approximate Retail Value (ARV): $575

Prize is non-transferable, and no substitution or cash equivalent will be made except at Sponsor’s sole discretion.

Winner Selection and Notification

The potential winner will be selected in a random drawing from all eligible entries received during the Sweepstakes Period. Drawing will occur on or about April 27, 2026.

The selected winner will be notified using the contact information provided and must respond within 48 hours to claim the prize.

If the selected winner:

• Cannot be contacted
• Fails to respond within 48 hours
• Is deemed ineligible

…an alternate winner may be selected.

Odds of Winning

Odds depend on the total number of eligible entries received.

California-Specific Disclosures

• California Civil Code Section 1542 Waiver:
• By entering, participants acknowledge and agree that they waive all rights under California Civil Code Section 1542, which states:
• “A general release does not extend to claims which the creditor or releasing party does not know or suspect to exist in their favor at the time of executing the release, which if known must have materially affected their settlement with the debtor or released party.”
• Publicity Release (California):
• Except where prohibited, participation constitutes winner’s consent to Sponsor’s use of winner’s name, likeness, city, and state for promotional purposes in any media without further payment or consideration.
• Dispute Resolution (California Venue):
• All issues and questions concerning the construction, validity, interpretation, and enforceability of these Official Rules shall be governed by the laws of the State of California, without regard to conflict of law rules. Any disputes shall be resolved exclusively in the courts located in California.

General Conditions

Sponsor reserves the right to cancel, suspend, or modify the Sweepstakes if fraud, technical failures, or any other factor impairs the integrity of the Sweepstakes, as determined by Sponsor in its sole discretion.

Release and Limitation of Liability

By participating, entrants agree to release and hold harmless Sponsor and its affiliates from any claims, damages, or liability arising from participation in the Sweepstakes or acceptance/use of the prize.

Privacy / Data Collection (California Notice)

Information collected will be used for marketing research, customer insights, and internal business purposes.

California residents may have rights under the California Consumer Privacy Act (CCPA/CPRA), including the right to request access to or deletion of personal information collected, subject to applicable exceptions.

For the name of the winner (available after April 27, 2026), send a request to Genesis of Ontario within 30 days of the Sweepstakes end date.
